Minority Stress and Sexuality Survey
Informed Consent
Thank you for considering participating in this research study on adult (18 or older) gay and bisexual men's minority stress and sexuality.
This study is being conducted by Dr. Darrell Greene, psychologist in private practice, and Dr. Paula Britton, of the Education and Allied Studies Department at John Carroll University. The purpose of the study is to better understand gay and bisexual men's concerns. We hope our results will better help counselors who work with gay and bisexual men. The survey takes about 20 minutes to complete. It begins with a demographic questionnaire, followed by questions about evaluation, expressing feelings, and sexuality.
The major benefit to you as a participant of this study is your contribution to our understanding of gay and bisexual men's sexuality as influenced by minority stress. All participants who complete the study will be asked if they would like to enroll by email in a random drawing, wherein participants can win 1 of a total of 10 $10 Amazon gift cards, or 1 $50 Amazon gift card, as a token of our appreciation.
There are risks to participating in internet-based research with regard to potential breaches of privacy or anonymity. Please be sure to close your internet browser once you have finished the survey to protect your privacy. In addition, you can further safeguard your privacy by deleting the webpage history from your browser after closing the surveylink.

You are also free to ask questions about your rights as a research participant by contacting the John Carroll University Institutional Review Board at (216) 397-1527. As an anonymous survey study, there will be no record of your participation. If you enroll in the gift drawing or request a summary of the study's results by email, your email will be kept in a safe file and will be erased immediately after the conclusion of the study or raffle. Your participation is voluntary. You may skip any questions you do not wish to answer. If requested, we will offer you information on counseling resources, but this is time-limited and not an offer of ongoing counseling.
We hope to include 600 gay and bisexual men in the study. We hope to publish and/or present the results of the study. As an anonymous study your survey responses will not be linked to your email or IP address. Only the researchers will have access to the participant responses. If you know of others who you think may be interested in the study and you feel comfortable in doing so, please feel free to inform them of the study.
CONSENT:
I have read the information given above. Dr. Darrell Greene and Dr. Paula Britton have offered to answer any questions I may have concerning the study. I understand that I must be gay or bisexual and at least 18 years old to participate. I understand that if I have more questions or concerns before or after participation in the study or about my participation as a research subject, I may contact them or the John Carroll University Institutional Review Board Administrator. I understand I may receive a copy of this form upon request. I understand I may request a summary of results once the study has been concluded. I understand the risks of participation in internet-research, and ways to protect my privacy. I understand that by continuing I have given my consent and agreed to participate.
