Official Rules

NO PURCHASE NECESSARY. A PURCHASE WILL NOT IMPROVE YOUR CHANCES OF WINNING. INTERNET ACCESS AND VALID EMAIL ADDRESS REQUIRED.

1. ELIGIBILITY. The survey (“Survey”) and associated prize drawing (“Drawing”) (referred to collectively herein, where appropriate, as the “Survey”) is open only to persons who meet all of the following criteria: (a) is a legal resident of North Carolina, South Carolina, Georgia or Virginia, exposed to MY FAVORITE DEALS offers in conjunction with his/her grocery shopping experience at a participating grocery store in one of the above-indicated jurisdictions; (b) received a non-transferable invitation to enter the Survey directly from the Catalina printer at checkout at his/her participating grocery store in one of the above-indicated jurisdictions (“Survey invitation”); and (c) is 18 or older. IF YOU DO NOT MEET ALL THREE (3) OF THESE CRITERIA, YOU ARE NOT ELIGIBLE FOR THE SURVEY (WITH THE EXCEPTION OF INDIVIDUALS WHO ARE 18 OR OLDER AND PARTICIPATE IN THE ALTERNATIVE METHOD OF ENTRY, AS SPECIFIED HEREIN): . Without limiting the foregoing, Survey is NOT open to the general public and directors, officers, and employees of Catalina Marketing Corporation (“Sponsor”), participating drug stores, and (as applicable) their respective parent companies, subsidiaries, and affiliates are NOT eligible.

2. SURVEY PERIOD. Begins January 30, 2015 at 12:00:01 am Eastern Time (“ET”) and ends March 15, 2015 at 11:59:59 pm ET (“Survey Period”). By participating, entrant agrees to abide by and be bound by these Official Rules and the decisions of Sponsor, which are final and binding in all respects.

3. TO ENTER. Go to the web site identified on the Survey invitation. As prompted, input all required registration information including your email address and the PIN number & Redemption Code from the Survey invitation. Be sure to input the PIN numbers exactly as it appears on the Survey invitation or else it will not be accepted. Complete the survey to the best of your ability and providing truthful answers to the questions regarding your experience using My Favorite Deals at Rite Aid in conjunction with your grocery shopping experience. (While survey must be completed, the content of your survey answers will not affect your ability to be eligible for the Drawing or your odds of winning a prize in the Drawing). Then, check the box indicating that you have read and agree to these Official Rules; entry will NOT be accepted if this box is not checked. Receipt of entries will not be acknowledged. Proof of transmission of entry does not constitute proof of receipt or proof of entry. Be sure to complete the Survey in a single online session as PIN numbers can only be used (by individual to whom it was issued via Catalina printer at checkout, as discussed above) to access the Survey once. To result in drawing entry, entry form must be received by end of the Survey Period. Limit of one (1) entry per person. If more than one (1) entry is received, all entries from such person will be disqualified. All entry information must be manually key-stroked by entrant and the use of automated, programmed or like means of entry is prohibited.

4. ALTERNATIVE METHOD OF ENTRY. If you did not receive an invitation and would like to participate in the Survey, please go to: www.research.net/s/RA1AME and use the following PIN Number and Redemption Code: PIN Number = 920846; Redemption Code = 49470. You will be eligible for the prize drawing if you participate via the alternative method of entry.

5. RANDOM DRAWING. Potential winners will be selected in a random drawing (the “Drawing”) from among all eligible entries received to be conducted by Sponsor on or about March 21, 2015. Potential winners will be notified via email and may be required to complete, sign and return an Affidavit of Eligibility, a Liability Release and a Publicity Release (where lawful) within an indicated time period. Failure of potential winner to timely return completed and signed Affidavit/Release (if applicable) or if potential winner cannot be contacted for any reason will result in disqualification and prize forfeiture, with an alternate winner being selected from among all remaining eligible entries received.

6. PRIZES/ODDS OF WINNING. Ten (10) Prizes: $100 Rite Aid gift card. The total value of all prizes in the Drawing is $1000. Gift card may be subject to terms and conditions of use as indicated thereon. No prize substitution; no prize transfer. Prizes are not redeemable/exchangeable for cash, except where required by law. Unspecified expenses in conjunction with acceptance/use of prize and taxes on prize (if any) are winner’s sole responsibility. Limit one (1) prize per family/household/address. Approximately 115,000 Survey invitations will be made available by Sponsor, but odds of winning a prize depend on the total number of eligible entries received.

7. RELEASES. By accepting prize, except where prohibited by law, winner agrees to the use of his/her name, address (city, state), likeness, voice, photograph and/or other indicia of persona for advertising/publicity/trade purposes by Sponsor and its designees without further compensation or notice. By participating, entrant indemnifies, releases and agrees to hold harmless Sponsor, participating retail grocery stores, their respective parent, affiliates, subsidiaries, and agencies, and the directors, officers, agents, representatives, shareholders, employees, successors and assigns of any of the above organizations from any and all liability arising from participating in the Survey, any Survey-related activity and/or acceptance, receipt, possession or use/misuse of any prize.

8. LIMITATIONS OF LIABILITY. PIN numbers are valid only for the person who received a Survey invitation via a Catalina printer at checkout to enter the Survey once; PIN numbers are not redeemable/exchangeable for cash and may only be used in accordance with these Official Rules. Any attempted transfer of Survey invitation/PIN number voids same and constitutes fraud (as discussed below). If PIN number contains a printing, mechanical, programming or other error, Sponsor’s liability will be limited to replacement with another PIN number (time permitting) provided that claimant establishes to Sponsor’s satisfaction (in Sponsor’s sole discretion) that he/she was the original recipient of Survey invitation; if this cannot be established to Sponsor’s satisfaction, Sponsor shall not have any liability of any kind in conjunction therewith. Sponsor is also not responsible for (a) any incomplete, incorrect or inaccurate information on Survey invitation, website or otherwise, whether caused by tampering, hacking, equipment malfunction/failure or other cause; (b) entries, transmissions of entry information and/or Survey invitations that are lost, late, illegible/garbled, corrupted, inaccessible, misdirected, incomplete or mutilated (with all such entries/Survey invitations being void); (c) any programming, technological, electronic, network, computer software or hardware or other error, malfunction or failure of any kind (whether affecting operation of Catalina printer at checkout and/or web site), regardless of cause; or (d) failed attempts to contact potential winners for any reason, including but not limited to attempted email transmissions being lost, corrupted or classified as “SPAM” or “junk” email. If the Drawing is incapable of being conducted as originally planned due to any reason beyond Sponsor’s control (such as, unauthorized intervention, fraud, natural disaster, governmental action, riot, strike, or other like circumstance), Sponsor reserves the right, in its sole discretion, to terminate, suspend or modify the Drawing and select prize winners in a manner deemed in its sole discretion to be fair, appropriate and consistent with these Official Rules. Notice of such action will be posted at the designated web site. Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds, in its sole discretion, to be tampering with the entry process or the operation of the Drawing or otherwise acting in violation of the Official Rules, with the understanding that disqualification (if applicable) shall NOT represent the sole remedy available to Sponsor and Sponsor shall endeavor to enforce its rights to the fullest extent allowable at law. (This includes but is not limited to any unauthorized use or other misuse of PIN numbers or Survey invitations and/or any act of fraud perpetrated by entrant or any third party). Sponsor’s failure to enforce any provision of these Official Rules shall not constitute the waiver of such provision.

9. PRIVACY. Entrant’s email address will only be used for purposes of Drawing administration and will not be used by Sponsor for any other purpose.

10. CHOICE OF LAW/DISPUTE. The federal or state courts situate in Pinellas County, Florida will have exclusive jurisdiction regarding any dispute, claim or cause of action regarding this Drawing or these Official Rules and such court will apply the laws of Florida (other than choice of law/conflict of law principles of Florida or of any other jurisdiction). Any such action must be brought individually, NOT as part of a class action; and, one’s recovery in such action shall be limited to actual costs involved in participating in the Survey with all other damages, rights and remedies being expressly waived. If any part of this provision is deemed by a court of competent jurisdiction not to be valid, legal or enforceable, such determination shall not affect the balance of these Official Rules and this provision shall be severed from the Official Rules and replaced with a substitute provision that is valid, legal and enforceable and most closely approximates the original intent of Sponsor.

11. WINNERS’ LIST. To receive a list of winners, mail a self-addressed stamped envelope to Catalina Marketing Services, ATTN: TJK 5th Floor Product Team, 200 Carillon Parkway, St. Petersburg FL 33716 by March 21, 2015. Winners’ list will be available after April 1, 2015.
