Note:  This form serves as a tool to create and submit inventory attachment records to the Reporting Agency's Asset Forfeiture Report.  Use this form to report up to 10 additional property inventory attachments.  You may add as many addendum reports as necessary to document all forfeitures which require an inventory report. 

You may update your report at any time as long as you use the original device on which the report was completed.  Upon submission you will have the opportunity to email all of the inventory attachments you have entered.  If you need a copy of this report for your records you may email Ms. Brina Gendhar at Gendharb@michigan.gov.
Note:  The inventory attachments required to be provided in this report only apply to property that was seized on or after February 1, 2016, and for which proceedings have been finalized for the purposes of appeal during this reporting period (January 1, - December 31, 2021).  Do not provide a description of property in this Attachment if either of the following apply: 

(1) The property was seized prior to February 1, 2016, but the proceedings were finalized for the purposes of appeal during this reporting period, or
(2) The property was seized on or after February 1, 2016, but the proceedings have not been finalized for purposes of appeal during this reporting period.
