Amaze and Diversity Council Australia (DCA) have partnered to develop survey questions for Australian employers to better understand the neurodiversity of their workforce. By collecting this data, your feedback will inform our guide supporting employers to start measuring neuroinclusion and diversity to drive positive change in their organisations.
This survey is voluntary and anonymous. Data will be treated in accordance with DCA's Privacy Statement. Data will be removed from SurveyMonkey after the consultation survey period has completed.

What will you be required to do?
This survey is seeking feedback from anyone aged 18 years and over who identifies as neurodivergent and/or having neurodivergent differences, as well as people working in diversity and inclusion-related roles on the wording and suitability of questions that a workplace could use to collect data on neurodiversity at work (e.g. in anonymous employee surveys).
- This survey contains 6 questions (not including demographics) and an additional 3 questions for people working in diversity and inclusion practitioner roles.
- Questions are a mix of multiple choice, multi check box, and open text.
- The survey will take approximately 7 minutes to complete.
Please note:
- The purpose of this survey is to gain feedback and insights to help us refine these questions.
- These questions are still being tested. They are not yet finalised or being recommended by Amaze and DCA as leading practice for employers.
