Thank you for your interest in having your facility participate in the Precious Drugs & Scary Bugs Campaign, an initiative led by the Illinois Department of Public Health (IDPH) Division of Patient Safety and Quality. This campaign aims to promote appropriate use of antibiotics, particularly for acute respiratory infections evaluated in outpatient care settings. Signing up for the campaign indicates your facility's dedication to the campaign's goals and to participating in the activities.

Campaign participation entails:
(1) Campaign sign-up
(2) Completion of a baseline and a follow-up survey related to perceptions, attitudes, and practices around antibiotic use
(3) Display of personalized commitment poster in exam rooms
(4) Optional:
---Participate in educational programs (e.g., webinars) as relevant
---Facilities are also encouraged to track data on provider antibiotic prescribing, and provide summary data to IDPH

To sign up, facilities first need to complete this short interest form to help IDPH plan and coordinate campaign logistics. This form should be completed by the facility leader (e.g., Medical Director). Only one interest form should be completed per facility.

The second (and final) step to campaign sign up will be completion of a brief commitment form online by each interested provider (i.e., prescriber) at participating facilities. This element of individual/personal commitment is essential to the campaign.
