1. Goal: Collect and share ACP Day initiatives across Ontario.

In light of all of the activity around COVID-19 many in-person events have had to be cancelled due to public safety concerns. This has led to organizations having to create innovative ways to recognize ACP Day.

With the uncertainty of how our health can drastically change over a short period of time, it is now more important than ever that we encourage communities to engage in Advance Care Planning.

Each year HPCO works with the Canadian Hospice Palliative Care Association (CHPCA) to catalogue the ACP Day activities across Ontario and the country. We would love to hear about your events and coverage. We invite you to share your successes with us in the comment box below. Please feel free to share multiple initiatives in as great detail as you would like to.

Please include the approximate number of people engaged, all partners you collaborated with, what was successful and what was not as successful, and any other relevant details of the initiative(s), etc.
