Audit of Special Educational Needs and/or Disabilities (SEND) - Early Years Provision 2026-27

Introduction

This is an audit to identify the children at your setting who require additional support for SEND.

This will enable the early years inclusion team to prioritise and allocate appropriate support and guidance.

Please complete for all children identified with SEND
Please answer No to question 5 if there are none and submit

Please complete this form by Monday 12th October 2026. You can make additional submissions if you have children that you identify with SEND later in the term.

The submission of this form assumes that parental consent has been sought and recorded to share this information.

Please make sure that your parents are informed and aware that this information is being shared with and held by the early years team.
