The REAP (Referral, Education, Assistance, and Prevention) program is a publically funded program for NH older adults (60 years old or older) and all residents of Federally funded Senior Housing sites. The program is provided at no cost to the recipient of services.  REAP is also available to caregivers and family members of older adults; and for housing professionals who are concerned about a resident or a housing site issue and seek assistance.

Housing professionals typically use the program in one of three ways:
  • Short-term Counseling for Resident. Housing professional initiates a referral to the REAP Program to provide individual, brief counseling to a resident. Counseling will provide assistance by working directly with an eligible resident who is experiencing a psychosocial difficulty that impacts their functioning or quality of life.
  • Technical Assistance for Housing Professional. Housing professional contacts REAP Program for assistance (problem-solving/information). Technical assistance will provide assistance by working directly with a housing professional who is challenged by a resident (s) or situation that impacts the housing site and residents: conflict between residents and or management, bullying, gossiping, hoarding, threat of eviction
  • Educational Sessions for Groups of Residents. Housing professional and REAP counselor initiates an educational session. Educational Sessions provide assistance by presenting information on a variety of topics of interest to residents and older adults. Also, Educational Sessions provide an opportunity for residents to meet their local REAP counselor.
