Encuesta sobre las necesidades comunitarias de salud en Central Oregon 2023

This survey will be used to help the health care and social service network write the Central Oregon Regional Health Assessment. A Regional Health Assessment is a report that identifies the key health needs, weaknesses and strengths of a community. It is developed by looking at data and asking community members for their input. Your voice and experiences are critical in helping us describe what is going on in the Central Oregon region (Crook, Deschutes, Jefferson, and Northern Klamath Counties; and the Confederated Tribes of Warm Springs sovereign nation).

 

Information from this survey will be used to inform community health improvement plans and how money is invested in community health and well-being. Some examples may be:

·         If a Regional Health Assessment shows that community members need more accessible health services including language interpreters, this could result in expanded interpretation services; or

·         If communities highlight housing as one of its greatest needs, this might result in investments in an affordable housing project.

 

This survey is an important opportunity to hear what you would want to see in place to support health for everyone. The Regional Health Assessment report will be available to the community in early 2024.

 

Filling out the survey is your choice – it is completely voluntary. All answers are anonymous and not connected to your name. Your answers will not be released, nor will they be shared with the public, or anyone else. Information shared with the public will only be a combined summary of responses. We will not be able to identify the people who took the survey.

 

This survey will take approximately 10 minutes for you to complete. At the end of the survey, you can  enter into a drawing for items such as iPads, $100 gift cards for groceries, and more.
