Central Oregon Community Needs Survey

Welcome to the Central Oregon Ongoing Community Health Survey. We are excited to hear and learn from you. The information from this survey will help community organizations across Central Oregon (Crook, Deschutes, Jefferson and Northern Klamath Counties; and The Confederated Tribes of Warm Springs sovereign nation) to better understand the health needs in your community. This knowledge will be used to support programs that may provide more choices to improve your health or the health of people you love. We can better understand community needs by hearing the voices of community members like you. We want to hear about the issues that you feel are the most important.

REMINDER:

You must be 18 years old or older to complete this survey.
The responses that you provide will remain anonymous, which means we cannot connect your responses to you in any way.
Your participation in this survey is completely by choice.
You are invited to retake this survey once a year so we can learn from you how things might be changing in your community.
This survey will take about 15-20 minutes to complete, and we value what you have to say in each section.
Community results will be available and shared on the Central Oregon Health Data website.
