Thanks for your interest in participating in our upcoming user research sessions!
This short survey is meant to verify your teen's eligibility and availability. (Teens MUST be 13-18 to be considered for this study.)  Please submit this survey ONCE per teenage child.  If you are selected, we will be in touch shortly via telephone or email. 

PLEASE COMPLETE THIS SURVEY WITH YOUR TEEN, AS PARENTS WILL NOT BE ABLE TO ANSWER MOST QUESTIONS.

More details:
- 1 hour conversation in our office with a Yahoo researcher at our office in San Francisco or in Sunnyvale
- If the teen is 13-17, both the teen and a parent/guardian MUST be present for the study
- If selected, we'll ask that you sign our standard Non-Disclosure Agreement prior to the start time of the user study
- Those who are selected and participate will receive a gift card dependent on the length of the user study.  Both the parent/guardian AND the teen will receive a gift card. (60 minute user study - $100 VISA Reward Card).
