You are invited to participate in a pilot study evaluating a self-assessment tool focused on Equity, Diversity, Inclusion, and Accessibility (EDIA). You have been selected because you are a practicing health professional in Canada. This tool is being conducted by the Equity in Health Systems (EqHS) Lab at the Bruyère Research Institute, under the supervision of Dr. Jerry Maniate (jmaniate@toh.ca). The study is funded by the Employment and Social Development Canada Federal Grant, through The College of Family Physicians of Canada’s Team Primary Care Project. Objectives and Summary:
The purpose of this pilot study is to assess the feasibility, utility, and initial psychometric properties of the EDIA Self-Assessment Tool. The tool is designed for self-reflection and is expected to take approximately 45 minutes to complete. However, participants may choose to spend more time if they wish.
Your participation in this survey is entirely voluntary. You are free to skip any questions or withdraw from the study at any time. If you choose to withdraw after submitting the survey, you can request to have your responses removed from the data by notifying the researcher within 14 days.

Risks and Benefits:
This study does not expose participants to physical risks. It is possible that the process of self-reflection could elicit psychological/emotional responses, however individuals are free to end their participation at any time and can provide feedback regarding any potential harm done through participation in the tool. Individuals will not be identifiable and data will be aggregated prior to analysis, so the risk of privacy concerns is low to none.

Confidentiality and Data Storage:
Your personal information will be treated as confidential, though absolute privacy cannot be guaranteed. Research records that identify you may be inspected by the Bruyère Research Ethics Board for auditing purposes. The results of this study may be published, but your identity will not be revealed unless you provide explicit consent. De-identified data may be shared with other researchers for verification or to further expand on our findings.
All research data will be securely encrypted at the Bruyère Research Institute. After the study concludes, your anonymized data will be retained for a minimum of 10 years.
