Welcome to the FSR Patient Registry COVID-19 Survey

Sarcoidosis is a disease of immune dysregulation that most frequently involves the lungs. This survey is trying to determine:
1.     How frequently sarcoidosis patients are infected with COVID-19 (corona virus).
2.     If a sarcoidosis patient is infected with COVID-19, what happens to them.
3.     Does receiving sarcoidosis medications prior to getting COVID-19 correlate with more severe illness.

This sub-survey is part of Foundation for Sarcoidosis Research's larger patient reported registry, FSR-SARC, and the primary consent comes under the registry. We received approval for this modification through our IRB for these additional questions as part of our registry. All participants of this sub-survey have:
1. Completed the Registry Consent form, which includes asking if participants have read Understanding Your Participation
2. Additionally consented to being contacted for additional follow up questions (such as this sub-survey) and other research opportunities outside of FSR.

However, you are under no obligation to complete this additional survey as a FSR-SARC participant.
