You have been invited to participate in a study titled “Training assessment for educational intervention on organ donation at DMVs in New York State.” This study is being conducted by Dr. Ashley Anker and Dr. Thomas Feeley at the University at Buffalo.

Volunteer Status
Your participation in this study is completely voluntary. Refusal to participate will involve no penalty or loss of benefits to which you are otherwise entitled as an employee of the New York State Department of Motor Vehicles [DMVs]. You have the right to refuse to answer particular questions or stop answering questions at any time.

The purpose of this research is to understand the experiences of Motor Vehicle Representatives [MVRs] in speaking with DMV customers about organ donation. Your responses on this survey will be used to help form a training program for MVRs on the topic of organ donation.

You will complete a survey regarding your experiences with donor registration as they pertain to your role as a MVR. You will be asked a series of questions about your experiences in learning about organ donation, your prior interactions with customers, and whether there is any information on organ donation that could assist you in discussing this topic with customers. You are free to skip any question you do not wish to answer.

Time Commitment
Your participation will take approximately 15 minutes.

There are no known risks to participating in this research.

This study will assist the project team in designing a training program for MVRs. As your DMV office may elect at a later date to participate in this training program, completion of the survey can allow you to contribute to the development of the program’s educational contents.

Upon completing the survey, you will be asked to provide your contact information, such that you can be awarded a $5.00 gift card to one of three organizations (i.e., Starbucks, Tim Horton’s, Wal-Mart). If you wish to discontinue your participation in the survey, you may stop answering questions at any time and may still provide us with your contact information for receipt of the gift card.

Your individual privacy will be maintained in all published and written data resulting from the study. After downloading information from the online survey, your contact information will be stored separately from your responses. Once gift cards have been mailed, your contact information will be destroyed. Thus, any answers you provide are ultimately stored anonymously, meaning they do not include your name or any other identifying information.

If you withdraw from this study, no further data will be collected but any information that you have provided may be retained by the researcher and analyzed.

In order to monitor this research study, representatives from federal agencies such as NIH (National Institutes of Health) and OHRP (Office of Human Research Protection) or representatives from the UB Human Research Protections Program may inspect the research records. This process may reveal your identity.
