1. About You - All Information is Kept Confidential

Your personal experience could help shape clinical research studies. Clinical research studies are an important step in the development of new medicines. When medical conditions have few treatment options available or when breakthrough treatments are being tested, clinical research studies become even more critical. This is the case with interstitial cystitis/painful bladder syndrome/bladder pain syndrome (IC/PBS/BPS) - otherwise known as urologic chronic pelvic pain syndrome.

You can help by participating in this brief survey as someone living with IC/PBS/BPS. Taking this survey is voluntary and your insights and opinions are appreciated, as the results from this survey will be analyzed and shared with the clinical research team.

If you know someone who may want to take this survey, you can share it with them by sending this link www.BladderPainStudy.com

The survey will take up to approximately 10-20 minutes to complete.

If you would like to be informed when the clinical research study is open for enrollment in your area, please leave your contact details at the end of this survey. In accordance with privacy laws, your contact details will be kept confidential.

We thank you for your support and assistance as we strive to develop new and better medicines for IC/PBS/BPS.

This survey is voluntary and intended for those living with IC/PBS/BPS.
