The Purpose Of This Questionnaire
No individual names or Company names will be identified or disclosed. We will discuss only trends and conclusions. We will also discuss our process for reviewing or defining a Company's transition plan as part of the Center for Business Transition.
Completing The Questionnaire
Participants will be requested as a pre-seminar activity to complete the questionnaire one week prior to the seminar.
Presenters will summarize the survey results in a format to be shared with the participants. Or, presenters will work the survey results into their overall seminar presentation and distribute the complete results at the end of the seminar.

Questionnaire Design
The pre-seminar questionnaire is designed to gather participant and Company information into five transitional areas of focus. Depending on the question, it will either be answered with a "Yes" or "No" or using a 1-5 scale indicating the degree of interest or agreement.

A pre-conference questionnaire can also be used with individual owners of companies who have expressed a need to review their current transition situation.
