Note: This form serves as a tool to create and submit inventory attachment records to the Reporting Agency's Asset Forfeiture Report. Use this form to report up to 40 additional property inventory attachments. You may add as many addendum reports as necessary to document all forfeitures which require an inventory report.

Note: The inventory attachments required to be provided in this report only apply to property that was seized on or after February 1, 2016, and for which proceedings have been finalized for the purposes of appeal during this reporting period (January 1, - December 31, 2022). Do not provide a description of property in this Attachment if either of the following apply:

(1) The property was seized prior to February 1, 2016, but the proceedings were finalized for the purposes of appeal during this reporting period, or
(2) The property was seized on or after February 1, 2016, but the proceedings have not been finalized for purposes of appeal during this reporting period.
