In-Person Student and Staff Survey
In-Person Student and Staff Survey
The County’s Mandatory Directive for Schools requires all schools to provide the County Public Health Department with a regular report on the number of students and staff anticipated on campus for an instructional program and the number of students and staff anticipated on campus for a non-instructional program. The following activities are instructional programs: in-person instruction, limited instruction, targeted support services, distance learning support (i.e., distance learning hubs), and facilitation of distance learning (e.g., teachers providing remote instruction from their classroom). Non-instructional programs would include athletics, childcare, and before- and after-school programs.
Each school must complete this survey by the end of the 15th of each month. A separate submission must be made for each school. In addition, each school must update its response within one week of a significant change in the number of students and/or staff on campus. A significant change is defined as a change of >5% or 20 individuals on campus.
Each school must submit this survey even if there are no students or staff on campus. This survey may be completed by either a school site administrator or a COVID-designee. However, if the survey is completed by someone other than the COVID-designee, he/she remains responsible for ensuring on-time submission.
