Introduction
This survey is being hosted by the Lionheart Foundation to determine whether our youth mental health services may be needed in the Foothills. The survey will take about 5 minutes to complete.

This survey is voluntary and confidential - meaning your personal information will not be connected to your answers.

By filling out this short survey you will be helping us assess the mental health counselling needs of youth in your community.

Optional: At the end of the survey you will be invited to share your contact information through a secure link, if you would like a follow up call or email.

Lionheart is a non-profit organization that provides adolescents and young adults, aged 12 to 25 years old, affordable access to specialized mental health counselling.
