Previous Disability Assessment Report

Until April of this year, a "Disability Assessment Report" form was used to provide medical information to Employment and Income Assistance. You can view a copy of the form here.

New Disability Impact Assessment

Starting in April of this year, a new "Disability Impact Assessment" form replaced the old form. While it appears longer due to a more accessible larger font size, the form was redesigned to simplify the form and eliminate some the questions for physicians.
