Children with Disabilities Survey

This survey is intended for parents and/or carers of children and young people with special education needs and disabilities (SEND) who access Children with Disabilities Services, Home Care, Continuing Care, Direct Payments, Dynamic Support Register and Short Breaks. 

When answering the survey questions, please let us know about your experience as a parent/carer. There will be other opportunities for children and young people where we will be asking them to give us their views.
