Background

Health centers who implement the Patient Centered Medical Home (PCMH) model of care engage in a period of practice transformation. This transformation engages team members at all staff levels in a process by which the health center redesigns aspects of their care model and daily processes. However, PCMH is never really "complete" or "finished," even after a center receives the recognition award by an external agency. The challenge lies in not only achieving that recognition but also maintaining the processes, practices, and model of care that were built during preparation for recognition.

Community health centers have utilized a variety of strategies for maintaining PCMH recognition and also maintaining the PCMH culture. The purpose of this brief survey is to quickly generate a list of these 'promising practice' strategies so we can share them across our organizations.

Please use the space below to share one good thing that your organization has done to maintain PCMH recognition, maintain the "culture" of PCMH, and/or continue staff engagement in the PCMH model.  The strategy you share could be large or small.  It could be something people said or did.  It could also be a specific practice change, or policy change. The only criterion is that it involved maintaining PCMH.
