RESEARCH CONSENT AND AUTHORIZATION

TITLE: Noetic EXperiences Archive (NEXA)

PURPOSE: This study aims to collect specific and remarkable noetic experiences that many people have to support future research on this topic. Some examples of noetic experiences include mind-to-mind communication, knowing the future before it happens, and communicating with a deceased family member. You must be an English-speaking adult (18 years or older) to complete this survey.

PROCEDURES: Your participation will take about 10-15 minutes. First, you will be asked to choose noetic categories that apply to your experience. You will then describe one specific noetic experience in your own words with free text. Then you will answer structured questions about its timing, location, characteristics, and any related outcomes. Some questions will vary depending on the type of experience selected. You may optionally upload documentation created before a related event or experience occurred, provided it contains no identifying information. You will then answer questions about yourself, such as how entering your experience impacted you, whether someone else witnessed your experience, your age, gender, education, race/ethnicity, and country where you live. You will also share your preferences about how your data may be used and whether you would like to provide contact information so the research team may contact you about your experience or for future research. If you do not provide contact information or include identifying information in your response or uploaded materials, your submission will remain anonymous. Please only enter ONE specific and remarkable experience per survey entry. Additional experiences may be submitted separately.

WHAT WILL HAPPEN TO THE DATA?: Your data will be kept indefinitely for future research at IONS. Anonymous data may also be released to other investigators for future research studies.

CONFIDENTIALITY: We will keep your information on password-protected computers on password-protected files.

RISKS: Although we have made every effort to protect your identity, there is a minimal risk of loss of confidentiality.

BENEFITS: You may or may not benefit from being in this study. However, by serving as a participant, you may help us understand more about noetic experiences. You will not receive your individual results from any research conducted from the archive. You may join the IONS newsletter list to learn about IONS general research results related to this data archive.

COSTS: It will not cost you anything to participate in this study.

COMMERCIAL DEVELOPMENT: By consenting to participate, you acknowledge that IONS may make any lawful use of your data, including, but not limited to, future research studies, destroying it, or transferring it to a public or private entity. We may use data obtained from you and stored in this research archive to make a discovery that could be patented or licensed to a company. There are no plans to provide financial compensation to you should this occur. However, should IONS ever provide your data to anyone else for research or commercial use, it will do so anonymously. Further, you will have no responsibility or liability for any use that may be made of your data.
PARTICIPATION: This research is being overseen by an Institutional Review Board (“IRB”). You may talk to the IRB chair Garret Yount at gyount@noetic.org if:
• Your questions, concerns, or complaints are not being answered by the research team.
• You want to talk to someone besides the research team.
• You have questions about your rights as a research subject.
• You want to get more information or provide input about this research.

You do not have to join this or any research study. If you do join and later change your mind, you may contact IONS at research@noetic.org to remove your contact information from the data. If you do this, we will keep your data anonymously. If you refuse to join or withdraw from the study, there will be no penalty or loss of any benefits to which you are otherwise entitled.
