This is a survey about school and health-related behaviors, experiences and attitudes. It includes questions about use of alcohol, tobacco, and other drugs; bullying and violence; and what you do at school and how you feel about it. You will be able to answer whether or not you have done or experienced any of these things.
In this survey the meaning of “alcohol use” is defined as drinking more than a few sips at a religious or family celebration.
This survey is completely voluntary: you do not have to answer these questions. However, your answers are valuable and will be very helpful in efforts to improve health and communication at your school. This survey is anonymous, which means that you will not be asked for your name and none of your answers can be linked to you in any way. Please read each question carefully. There are no "right" or "wrong" answers; just give your best estimate. If you do not wish to answer a question you may leave it blank and continue on.
