Portsmouth Clinical Commissioning Group and Public Health Portsmouth plan and buy health and wellbeing services on behalf of Portsmouth residents.

We are interested in hearing your views about our approach to tackling obesity in the city and helping shape the future of local weight management services (i.e. services which offer support or treatment to people who are overweight or obese (this does not include underweight)).  This will help to ensure service design takes into account the views of local residents.    

In this survey we're interested in hearing from all members of the public, whether or not you have accessed weight management services.  There will be a separate opportunity for professionals to provide their views.      

It will take about 10-15 minutes to answer these questions. We don’t need your name, as the questionnaire is anonymous.  Please answer by ticking your response or adding text in the spaces provided.  There is an opportunity at the end to add more comments should you wish.
