QUEST Common Indicator Set - Pilot Trial Evaluation Form 2024

Introduction

Thank you for agreeing to participate in the Pilot Trial of the Common Indicator Set of the WFOT Quality Evaluation Strategy Tool (QUEST). Your feedback will be used to evaluate and further develop this new WFOT resource.

The Common Indicator Set is a continuation of the work of WFOT on QUEST and includes a collection of measures to be used as a starting point for defining indicators for a particular occupational therapy service. The QUEST Common Indicator Set identifies 19 quality indicators applicable to services provided by all occupational therapists, regardless of geographic location, practice settings and populations served.

Your role in the Pilot Test is to identify quality indicators for an occupational therapy service using Worksheets developed for the QUEST Common Indicator Set. The occupational therapy service may be a current or former workplace or practice placement site. It is not necessary for you to have previously used QUEST in your work. After completing the Worksheets, we ask to complete a short survey on the following pages.
