Community Resource Card Order Form

Use this form to place orders for the accordion-fold wallet-size card containing resources for City of Alexandria residents who are unsheltered or in need. The Department of Community & Human Services covers printing costs. Please order only what you need. We will notify you when the order is ready for you to pick up.

8.For non-City government partners, we will contact you when the order is ready for pick-up at 4850 Mark Center Drive in the main lobby reception. Parking is free in the garage. Please leave the name of your organization and any comments below.
Please remember- You will pick up at 4850 Mark Center Drive, home to the Del Pepper Community Resource Center, when notified. Depending on the quantity and supply, cards will be ready in 1 to 5 business days.
