Introduction

This system facilitates the release of a patient's medical records held by Olympic Park Sports Medicine Centre to the patient directly or to a third-party. The system will:
  • ask you a series of questions and in some instances ask you to upload a document to enable your request to be processed; and
  • prompt you to pay the fee associated with your request. 
Please note - until all questions have been answered and the fee paid - your request cannot be considered. 

Required information/document from a patient

If you are a patient, to answer the questions you will need to: 
  • enter details from your Medicare Card and consent to us confirming these details with Medicare Australia; or 
  • upload a document that enables us to verify your identity (for example, a driver's licence or passport). 
We recommend you have your Medicare Card with you or an identity document saved on your computer before you start this request. 

Required information/document from a third-party 

If you are a third-party, to answer the questions you will need to upload a document that affirms the patient has authorised you to make this request (for example, a letter signed by the patient that permits us to release their medical records to you). 

We recommend that you have the authorisation document saved on your computer before you start this request. 

Fee

All requests attract a $33 fee to meet the costs we incur in:
  • assessing your request; and
  • collating and sending you the requested records. 
The fee is in Australian dollars and inclusive of Australian goods and services taxation (GST). 

After you submit your request, you will be emailed an invoice. Payment of this invoice can be made by Visa, Mastercard or Direct Debit and is required before your request can be processed. 

Timeline

After you have completed all questions and paid the fee, your request will be assessed. If we consider the release of the requested medical records to be appropriate or we are compelled under the law to release the requested medical records we will endeavour to do so within 21 business/working days from the date of the request. 

Withdrawal and refund

After a request has been paid, it cannot be withdrawn and fees cannot be refunded.

Security

The information you enter into this system will be encrypted - using secure socket layer protocol - in transit from your computer to the information and communications infrastructure our service providers and we use. Furthermore, both our service providers and we have deployed rigorous measures to protect the data we hold from unauthorised access.
