Sonoma County EII System Partner Survey

Thank you for your interest in completing this EII System Partner Survey! The inputs from this survey will be used to develop a clear and concise map of Sonoma County’s local Early Identification and Intervention (EII) System. The map will be shared at the March 2nd EII Community Visioning Session to support participants with: 1) understanding the current EII system of care (both community-based and clinical services); and, 2) ideating around how the current EII system can be further strengthened for families, especially those on the margins. The map may be further built out with more information as we gather that over the next couple of months.
