This survey will take about 5 minutes to complete, and will help determine if you qualify to participate in the focus group.

Due to research constraints, only a limited number of people will be selected to participate in the group. If you qualify, and are invited to participate, you will receive a $125.00 incentive for your participation for the entire duration of the focus group.

All of your responses to this survey will be kept in strict confidence and will only be used to determine whether you qualify to participate.
