The Church Cares Helper Post Training Questions (FA25.rev)

Stop & Reflect

Dr. Jennifer Ripley, a researcher at Regent University is conducting a program evaluation on The Church Cares. I understand the purpose of this research and the expected results. I understand that I will be asked to answer some questions.

My participation in this questionnaire should take a total of about 5 minutes. I understand that my responses will be confidential and that my name will not be shared with anyone. I know that I can choose not to answer any question asked and can stop participating at any time.

Any possible risks have been explained to me. I know that I can get more information by contacting Dr. Ripley at jennrip@mail.regent.edu or 757-352-4296, and the Regent IRB office Contact Info—(757) 352-5010; irb@regent.edu.

I confirm that I am at least 18 years old. My completion of the survey means that I am voluntarily participating in this project.
