Stop & Reflect

Dr. Jennifer Ripley, a researcher at Regent University is conducting a program evaluation on The Church Cares. I understand the purpose of this research and the expected results. I understand that I will be asked to answer some questions.

My participation in this questionnaire should take a total of about 5 minutes. I understand that my responses will be confidential and that my name will not be shared with anyone. I know that I can choose not to answer any question asked and can stop participating at any time.
