ACES Aware Trauma-Informed Principles and Practices Community Provider Survey

Dear Network of Care Partner:

The purpose of the survey is to determine how partners will incorporate the science of ACEs and toxic stress trauma-informed care principles and practices into their work with clients, with particular attention to outlining how all partners will provide care and support that is trauma-sensitive, healing-centered, and sensitive to the person’s racial, ethnic, cultural background, sexual orientation, and gender identity.

Please use the definitions below to select the rating that best describes the implementation status of each principle or practice in your Kings County healthcare or community service organization. Thank you in advance for supporting this important effort!

Definitions
(1) Not implemented. The organization does not address this principle or practice in our work with clients and does not have immediate plans to address.
(2) Planning to implement. The organization does not address this principle or practice in our work with clients, but has future plans to implement.
(3) Partially implemented. TThe organization does not address this principle or practice in our work with clients , but the principle or practice has not been fully implemented.
(4) Fully implemented. The organization universally addresses this principle or practice in its work with clients and monitors to ensure implementation.
