We have designed a prototype for a universal employer medical form to support most requests. This has been informed by an advisory group of physicians, employers, human resource professionals, and other experts.
This work has been guided by the following best practice anchors:
- Collect only essential information
- Focus on questions health professionals can reasonable answer
- Keep form as clear and simple as possible.
This consultation seeks your feedback about two prototype universal forms:
- Employer Information Form: an optional form employers can use to share with the physician or health care professional key details about the patient's job, workplace, and employer questions.
- Occupational Medical Form, to be completed by the physician or qualified health care provider capturing their medical assessment in a structured and consistent way.
