VA CoEPCE Case Studies Quality Improvement Questionnaire     Reference: www.va.gov/oaa/coepce

Welcome to VA's Centers of Excellence in Primary Care Education (CoEPCE) Case Studies quality improvement questionnaire. CoEPCE leaders work to partner with academic institutions and other public or private professional organizations to improve interprofessional education and collaborative practice.

Thank you for your willingness to provide feedback for this project. We hope to better understand the usefulness of the case studies in three areas:

Dissemination – Are these case studies reaching a broad audience? Who is reading them?
Effectiveness – In what ways is the information here useful to readers? How could it be made more useful?
Adoption/Implementation – Are readers adopting the strategies outlined in these case studies? How are they adopting them to their own contexts?

Please note: Information gathered in this assessment will be anonymous and is intended for operational program improvement.
