Wallace County Community Health Needs Assessment

This survey is being conducted in partnership with Greeley County Health Services, the Greeley County Health Department, and Wallace County Health Department. The results of this survey and other information gathered by the survey will be used to make recommendations to bring about improvements in outcomes related to health. The survey results, other information about the health care system, and recommendations for the future will be shared with the citizens of Greeley and Wallace County. If you have completed this survey before, please do not fill it out again.
The following information is provided for you to decide whether you wish to complete the survey. You should be aware that even if you agree to participate, you are free to withdraw at any time without penalty. Your individual responses will remain confidential; only total survey results will be shared. You are not obligated in any way to take or complete this survey. Completion of the survey indicates your willingness to participate in this project and that you are at least eighteen years old.
