HIV Stigma or Discrimination Incident Form - Newcastle Hospitals Staff You can use this form to report instances of HIV stigma and discrimination that have occurred in any Newcastle Hospital site or service.This may have happened to you directly, or to a loved one, or it may be something you have witnessed as a bystander. The information will help our HIV Confident Champion (Annie Laverty, Chief Experience Officer) and her team to understand where and why stigma is taking place and to take action to ensure it doesn't happen to others. You can choose to remain anonymous The report will be viewed by Annie and the Patient and Staff Experience team. If you do let us know who you are, you can later choose whether you want to be identified when we are dealing with the issue. If you are reporting as a staff member, we assure you that you will experience no detriment from highlighting issues. If you have given your details, Annie or a member of the Patient and Staff Experience team will call you within two weeks to discuss the experience and what you would like to happen next. They will signpost you to support, if needed. The senior staff where the incident occurred will be informed within one week, and education will be offered to the team to improve awareness and reduce stigma in future in line with our People Plan with targeted support from our Equality, Diversity & Inclusion team. Quarterly reporting of incidents will be shared with People Committee.
