Thank you for agreeing to take part in our survey. The goal of the study is to increase our knowledge of the post-placement needs of adopted children and their parents so as to improve adoption professional training and develop more effective services for adoptive families. The study is being conducted by David Brodzinsky, Ph.D. and Jeanne Howard, Ph.D. of the Donaldson Adoption Institute in New York City.

The questionnaire on the following pages should only take about 30-40 minutes to complete. The survey has been set up so that you can begin it at one time and complete it later. Simply make sure that you use the same computer and browser when continuing the survey.

We hope that you will respond in a frank and open manner. Please do your best to answer all questions. There are minimal risks associated with participation except possible discomfort with some of the questions and the possibility of breach of confidentiality. If there are any questions that you do not want to answer, simply skip them. In addition, should any question create emotional distress for you, we are available by telephone or email to discuss the issue and/or to help you locate someone to consult with in your community. Please note that only one parent per family should fill out the survey

To protect respondents' privacy, no identifying information is being requested; the survey is anonymous. All information collected will be kept on a password-secure computer. Once the study is completed and all online data have been transferred to our computer, the online database will be purged. Only summarized data will be used in reports, presentations, and publications; an individual's specific responses will not be included in these documents.

Your completion and return of the questionnaire indicates your consent to participate. Participation in the survey is strictly voluntary. There are no consequences for refusing to participate and you are under no obligation to take part in the study. Furthermore, you can withdraw from the study at any time without penalty. There are no direct benefits to you for participating, but we hope you will experience satisfaction knowing that your information may help inform the field about how best to serve adopted children and their parents.
