The web application is not designed or intended for use in the diagnosis or treatment of disease or other conditions, and the recommendations provided by the web application do not constitute medical advice. The web application will collect certain limited information to help inform the predictive modeling dashboard and identify areas where there is a rise in COVID-19 cases. This will not include any data that may personally identify you. Unless you provide your express consent within the web application, no other information will be collected.
