The Los Angeles Department of Mental Health (DMH) and Los Angeles County Office of Education (LACOE) have partnered to evaluate a strategy to organize school and community resources. We are asking all students from this school to complete this survey to help us improve the quality and impact of this strategy for school staff, parents/caregivers, and students. This survey should take approximately 15 minutes to complete and will help to create a better learning environment for everyone at the school.
Participation in this survey is completely voluntary. There are no penalties for declining to participate in the survey or for declining to respond to items on the survey by selecting "N/A" or "Decline to respond." Your survey response will not have any impact on your school records or family's eligibility for services and support.

This survey is confidential. You will be asked to create a unique ID below by answering a few questions. This unique ID will be used to link your survey responses across time to identify trends and better understand how, if at all, the Community Schools Initiative is having an impact. This unique ID cannot be used to determine your identity. Your responses will be combined with the responses from other individuals and will be reported in aggregate (group) form only.
