INTRODUCTION

This survey is being conducted by Information by Design (IbyD) on behalf of Dorset Council. IbyD are an independent market research company, they are members of the Market Research Society (MRS) and abide by the MRS Code of Conduct. This is the fifth time Dorset Council have conducted the survey. If you completed the survey previously, you can still complete it this year. We are interested in hearing everyone’s views.

Data Protection and confidentiality
We will hold your data in line with the data protection laws, and the General Data Protection Regulations. This means that we will treat your data confidentially, hold it securely and use it only for research purposes and to create reports. Data will be held by IbyD and you can see our privacy policy at www.ibyd.com/privacypolicy. We will also transfer data to Dorset Council who will retain it for six years or as long as required by law. Please check the consultation privacy notice which can be accessed on the website. You can withdraw your consent at any time by contacting either IbyD or Dorset Council. The data will not be transferred to third parties.

Question Title

The survey asks questions about the council and your local area, and also collects personal data.  This includes data on your physical and mental health, ethnicity, religion, gender identity and sexual orientation, which are optional questions.  To confirm that you agree to take part in the survey and to the use of your data as described above, please tick the box below.
