The Home Health Value-Based Purchasing (HHVBP) Model Technical Assistance (TA) Team invites you to participate in a brief survey. We hope to learn more about your home health agency’s (HHA) experience with the resources provided for the expanded HHVBP Model and identify areas for future outreach and education. Your input in this survey is vital and appreciated.
Who should respond? We would like to hear from anyone actively involved in your HHA’s expanded HHVBP Model efforts. We encourage you to share the survey link with others. The survey can be completed by one (1) individual or multiple individuals, or you may find it helpful to assemble your HHA’s expanded HHVBP Model team and complete the survey as a group. Completing this survey is voluntary and anonymous unless you choose to provide your contact information at the end.
How will the data be used? The survey data will be used by the Centers for Medicare & Medicaid Services (CMS) to shape the development of future events, communications, and educational products to support HHAs' implementation of the expanded Model. The survey will cover the following topics:
