The Minnesota Department of Transportation (MnDOT) Worksite Cooperation Committee would like to better understand the experiences of workers in the highway heavy construction industry.

Your responses are completely anonymous and will be compiled by select staff from MnDOT in aggregate. Your answers will NOT be associated with you personally.

The survey will take around 5-10 minutes to complete. The results of the survey will help inform industry stakeholders and help create tools to improve worksite culture.

Note: To the extent information submitted in connection with this survey identifies an individual or business, the information may be protected from disclosure under the Minnesota Government Data Practices Act (MGDPA). Data will be used by MnDOT’s Office of Civil Rights and the Worksite Cooperation Committee to identify barriers and concerns in the highway heavy construction industry. Any identifying information you provide will not be associated with your individual survey responses. However, any identifying information will be accessible to MnDOT employees whose job reasonably requires access to the information, to anyone you specifically authorize to receive the information, pursuant to a court order, and by any other person or entity authorized by state or federal law. You are not obligated to respond to this survey, and there are no consequences if you do not respond. However, if you do not respond, MnDOT’s Office of Civil Rights and Worksite Cooperation Committee may not fully understand the highway heavy construction industry’s experience.
