My Health Focus is a new online platform being developed to provide clear, trustworthy health information, share patient experiences, and create opportunities for people to learn, connect, and make informed decisions about their health.

This short survey is designed to help us understand what you would find most useful, so that My Health Focus can be shaped by the people it is intended to support.

We want to make sure My Health Focus focuses on the things that matter most to you, not assumptions. Your feedback will help guide what we build and how we build it.
