National Survey of Clinical Services and Training for

Instructions for Survey Completion
This survey should be completed for each specific clinical site by the clinical staff professional responsible for the assessment and treatment of Obsessive-Compulsive Disorder (OCD) and/or by the Director of Training for academic sites. Please respond for the setting at which you spend the most time, that is, your primary site. Questions refer solely to service and training for OCD (not for related disorders). This survey will take approximately 30 minutes to complete. To submit your site information please click Submit Survey.
The overall results of this survey will be presented and/or published in a format that ensures individual sites are not identifiable. If you wish any information about your site services to be included in a database of services in Canada for OCD and posted on the CIOCD website (www.ciocd.ca), please complete the authorization section at the end of the survey.
