PFA Consultation - Neurodegenerative Conditions

Thank you for taking the time to open this questionnaire and taking part in this important PFA survey. In sending out the survey we are hoping to further identify the number of former players who have been diagnosed with a neurodegenerative disease. We want to obtain a more informed position regarding former players and their families' needs, so both the Union and wider industry can develop and improve our support services available.

 
Aims of the Consultation
 
The results of this survey will help the PFA's call for an industry wide care fund, which includes a commitment of financial support for families from the wider football community including the Premier League, The Football Association (FA), English Football League (EFL) and League Managers Association (LMA).

We also hope to be able to reach former players or their families who are currently living with a neurodegenerative disease, or have concerns about their loved one, but have not yet accessed support. So please share this survey with other former players and their families, including squad WhatsApp groups and Facebook groups. 

 
Data and Results
 
The data you submit will be kept confidential and held securely by the PFA, with no personally identifiable information being sent to third parties. In order to further discussions around an industry wide fund and increased support for former players and their families, we will send aggregated and anonymised data to football's stakeholders including the Premier League, The FA, EFL, LMA, FIFPRO, International Football Association Board (IFAB), UEFA and FIFA. We may also make overall figures and findings public to increase awareness of the links between football and increased prevalence of neurodegenerative disease in former players and also to help improve protections within the game for current and future players.
