We are interested in learning what you know about alcohol use during pregnancy; and any screening, brief intervention, or harm reduction practices you use in your workplace.  This will help us to identify training, education and resources to better meet the needs of those working in this field.

There are 5 sections of questions:

 ·         Section 1: Knowledge

·         Section 2: Screening and Brief Intervention

·         Section 3: Attitudes and Beliefs

·         Section 4: Training and Education

·         Section 5: Demographics

Depending on the type of service you offer (i.e., primary care, community based support), some questions will be a better fit than others. We encourage you to answer to the best of your abilities.

The survey questions have been developed by a multidisciplinary research team that included representatives from the Society of Obstetricians and Gynaecologists of Canada (SOGC); the College of Family Physicians of Canada (CFPC); Justice Canada; the Canada Fetal Alcohol Spectrum Disorder Research Network (CanFASD); Mothercraft/Breaking the Cycle and researchers in the field from the Centre of Excellence for Women’s Health; University of Calgary, University of British Columbia, University of Alberta, University of Toronto, Guelph University, University of Saskatchewan and the University of Regina and the Public Health Agency of Canada.

The purpose of this survey is to obtain information that will inform the development of targeted education and resource initiatives around alcohol use.  The survey has received ethical approval from the University of Ottawa Research Ethics Board.  All data will be anonymized and access will be restricted to only members of the research team.  The survey should take approximately 15 minutes to complete and as a thank you, you have the option of entering your name into a draw to win an iPad.

Your participation in this survey is voluntary and you are free to withdraw from the survey at any time by not submitting the survey.  Should you have any questions about the survey, please contact Ms. Kyla Kaminsky, Research Assistant by email or phone 1-800-561-2416 ext. 226.  Please note that by clicking "submit" at the end of the survey, you are indicating that you have read and understood the above information and are consenting to participate in this research project.
