Communities of practice are groups of people who share a concern or a passion for something they do and learn how to do it better as they interact regularly. The Rural Hospice Palliative Care Community of Practice (HPC CoP) was created to facilitate discussions, networking, information sharing and two way communication between individual CoP members with an interest in rural hospice palliative care and HPCO to support shared dialogue and opportunities through the CoP.

  • To create synergy and identify common projects
  • To enhance capacity
  • To promote best practice guidelines
  • To identify and communicate system issues related to rural hospice palliative care
  • To develop as an active and engaged community of practice advancing the goals as defined by its members.
