The Swallows - Psychological Support Service - Survey

The Swallows is exploring whether to introduce a dedicated psychological support service for people affected by head and neck cancer: patients, caregivers, family and friends.

This short survey will help us understand what psychological support should look like. There are no right or wrong answers, and you can skip any question you'd prefer not to answer.

Your responses are anonymous and will only be used to help plan services. Before you begin, please read and respond to the consent statements below.
1.I consent to Unboxed Psychology collecting and using the information I voluntarily provide, including personal data, for the purpose of understanding need and planning support services.(Required.)
2.I consent to Unboxed Psychology storing my anonymised survey responses for 12 months for analysis purposes.(Required.)
3.I consent to my anonymised survey responses being used in reports, presentations and funding applications to support the development of this service.(Required.)
