INTRODUCTION: The tornado we experienced has been troubling for many of us. Because of this, we want to take the time to gather information that will help us determine better ways to help you.
We would like to ask you some questions about what happened and maybe ask about your feelings and thoughts about the tornado. We want you to think about how you have felt during the past couple of months and decide how each of the following statements describes you. You will choose whether it has bothered you, "not at all", "a little bit", "kind of", "a lot", or "very much".
