About this survey
Relationships Australia South Australia (RASA), on behalf of the SA Pathways Network, is seeking some detailed feedback from you about your experiences of using the Pathways KIOSK service. Your feedback is valuable to us - by providing your views on the KIOSK, you will help us improve the service we provide. No special knowledge is required to answer these questions and there are no right or wrong answers; we are interested in your opinion. The evaluation form asks you to provide us with (1) information regarding your experiences with using the KIOSK, (2) your opinions regarding how the KIOSK can be improved, and (3) basic demographic information. We expect the survey to take no longer than 10 minutes to complete.
From your feedback we will continue to refine and improve the way we deliver the KIOSK. To do this effectively, we will analyse all the anonymous information we gather during this evaluation process, and report the findings to our funding bodies. We may also use the data analysis in academic journals and conferences to share our findings and learning. This is an anonymous evaluation process. Your answers will be kept confidential and you will not be identified in any of the results. All data are reported at group level. By completing the following survey you are consenting for RASA to use your de-identified information in their analyses. If you choose to take part in this survey, you should be aware that you are able to stop completing the survey at any time you wish by simply closing your browser.
